Questionnaire, Photo, Measurements There is a simple questionnaire for you to complete when you place your order. The questionnaire will ask you about your costuming needs, performance venues, and dance style so Anja can create the perfect costume 'just for you'. Please e-mail a photo of yourself. A photo of yourself in one of your favorite belly dance costumes would be ideal. Request Questionnaire and Measurement Form Payment 50% deposit ($397.50) is due when you place your order. The balance ($397.50 + 15.00 Shippping) is due within 30 days. |
